At Acme, we help you plan for what is coming, deal with what is happening now, and connect you with the services and resources you need. We work with your coordinator, your family, and your allied health team to make sure the transition is coordinated, not chaotic.
Transitions we support
School to post school
Exploring employment, further education, volunteering, and community participation. Resume building, interview preparation, workplace orientation, and connecting with disability employment services.
Moving out of the family home
Independent living skills training, SIL assessment and setup, property visits, gradual transition plans, and emotional support for both the participant and their family. A huge milestone that we handle with care.
Ageing and changing needs
As participants age, support needs may change. We help with reassessing care plans, adjusting services, connecting with aged care supports where appropriate, and ensuring continuity of NDIS funding through plan reviews.
Relationship and family changes
Changes in relationships, family structure, or living arrangements can impact support needs. We provide person centred support to navigate these changes and adjust plans accordingly.
